New patient paperwork 

Simply click on the box in the top right corner and it will open the forms in a new tab. This will allow you to print the packet or fill out online. If you choose to fill the packet out online, be sure to sign appropriately once printed. Please read carefully and fill out entirely. You can return the packet to our office or email to admin@southernmedicine.com. Once you return the paperwork, we will review & schedule accordingly. Please call with any questions or concerns.
